Cloud: Log in to Jira as an administrator, then navigate to Jira settings > Apps > Find new apps. Search for ProForma then click Try it free.
Server: Log in to Jira as an administrator, then navigate to Administration > Add-ons or Manage apps. Search the Marketplace for ProForma then click Install. A popup confirming successful installation should appear after a few moments.
After installing ProForma, you may find that some modules are disabled depending on the Jira product and add-ons you use. For instance if you do not use Jira Service Management, or have Automation for Jira installed, some modules will be disabled. This is expected and does not require any further action.
If ProForma is no longer required, it can be removed from a Jira instance. To uninstall ProForma, log in to Jira as an administrator, then navigate to Administration > Add-ons or Manage apps. Locate ProForma and click Uninstall. Note that you will not be able to view forms on issues once ProForma is uninstalled.
If your ProForma license/subscription expires you will not be able to create new form templates, or add forms to an issue. Depending on your Jira instance, you may or may not be able to view existing forms. Currently, Cloud instances using the “new Jira issue view” will not display any ProForma forms once the ProForma license expires. In Server instances and Cloud instances that use the old issue view, you will be able to see existing forms on issue, but not add or edit forms.
If you no longer wish to maintain a ProForma license/subscription and you have been using ProForma for some time, we recommend that you downgrade to ProForma Lite. While you will not be able to create new forms, this will ensure that existing forms on issues continue to be available.
The data for each form attached to an issue is stored as a hidden property of that issue and is not automatically deleted when ProForma is uninstalled or disabled. If you do decide to uninstall ProForma, provided that the issues have not been deleted from Jira, all of the form data should still be accessible. You simply need to install ProForma again and enable it for the relevant project. You will then be able to view existing forms on issues, download forms from the relevant issues or use the spreadsheet download under Project Settings > ProForma Forms.

By default, ProForma captures anonymous usage analytics from all users, both in Cloud and in Server. To protect privacy, ProForma's analytics have been configured to never capture the following information:
Personal information, such as account names, email addresses or other user information.
Form contents: Both the written responses and the questions on the form themselves
We use analytics data to better understand which ProForma features are most frequently used, and how we can make improve our product. We also use this data in designing our onboarding system (Intercom), so that we can provide the right guidance at the right time for our users.
A hash of an accountId provided by Jira is used to identify individual users. This ID does not contain personally identifiable information.
In order to ensure transparency, all of our analytics data is captured through the browser, so that you can verify for yourself that we are only collecting what we say we collect. A sample of the analytics data is available below. All of our analytics data is captured via analytics.thinktilt.net.
